Disclaimer: Dr. Samantha Dessources (Dr.Sammy) received her Doctor of Medical Science degree from the University of Lynchburg in 2022.  Her use of “doctor” or “Dr.” in relation to herself solely refers to that degree.

Dr. Sammy is a Board Certified Physician Assistant, licensed in the states of New York and Georgia. Although she maintains active licenses, she does not currently practice medicine, so she can focus on educating and coaching clients as a full time activity.  Dr. Sammy, is a certified health coach via the Dr. Sears Wellness Institute, and aims to share evidence-based knowledge and raise awareness about managing PCOS symptoms. Product Agreement for Digital eBook Wellness Journal “Wings of Faith: Wellness Journal”

1. Introduction

This Product Agreement ("Agreement") is a legally binding contract between you ("Customer," "you," or "your") and Zai-Teo Wellness, LLC ("Company," "we," "us," or "our") concerning the purchase and use of the digital eBook wellness journal ("Product"). By purchasing and using the Product, you agree to be bound by the terms and conditions of this Agreement.

2. Purchase and License

2.1 Final Sale and Non-Refundable: All sales of the Product are final. Once the Product has been purchased, it is non-refundable.

2.2 Non-Transferable: The Product is licensed to the original purchaser only and is non-transferable. You may not sell, distribute, or transfer the Product to any third party.

2.3 Personal Use Only: The Product is for your personal use only. Any commercial use, reproduction, distribution, or public display of the Product, in whole or in part, is strictly prohibited.

2.4 Prohibition on Sharing: Sharing, redistributing, or posting the Product, in whole or in part, in any form or by any means, is strictly prohibited.

3. Compatibility

The Product is designed to be compatible with various note-taking apps, including GoodNotes. GoodNotes is a trademark of Time Base Technology Limited. Our Product is not endorsed by or affiliated with GoodNotes or Time Base Technology Limited.

All intellectual property rights in and to the Product, including but not limited to copyrights, trademarks, and trade secrets, are owned by Zai-Teo Wellness, LLC. You acknowledge that you have no rights in or to the Product other than the limited license granted herein.

6. Limitation of Liability

To the fullest extent permitted by law, Zai-Teo Wellness, LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses, resulting from:

  • Your use of or inability to use the Product;

  • Any unauthorized access to or use of our servers and/or any personal information stored therein.

7. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of New York and the federal laws of the United States of America, without regard to its conflict of law principles.

8. Severability

If any provision of this Agreement is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall be enforced to the fullest extent under law.

9. Entire Agreement

This Agreement constitutes the entire agreement between you and Zai-Teo Wellness, LLC regarding the purchase and use of the Product and supersedes all prior agreements and understandings, whether written or oral, relating to the subject matter hereof.

10. Amendments

Zai-Teo Wellness, LLC reserves the right to modify this Agreement at any time. Any modifications will be effective upon posting the revised Agreement on our website. Your continued use of the Product following such changes constitutes your acceptance of the new terms.

By purchasing the Product, you acknowledge that you have read, understood, and agree to be bound by this Agreement.
